Nexterone Dosage

Given as a slow injection into a vein, Nexterone is used to help control irregular heart rhythms. Your Nexterone dosage is calculated based on the severity of your arrhythmia, how you respond to the drug, and various other factors. The doses are usually given in a hospital, where you can be carefully monitored and the amount can be adjusted as needed.

An Introduction to Dosing With Nexterone

The dose of Nexterone® (amiodarone premixed injection) your healthcare provider recommends will vary, depending on a number of factors, including:
 
  • The severity of your arrhythmia
  • How you respond to the medication
  • Whether you develop side effects.
 
As is always the case, do not adjust your dosage unless your healthcare provider specifically tells you to do so.
 

What Is the Recommended Nexterone Dosage?

The recommended initial dosage of Nexterone is usually 1000 mg given as a continuous infusion (a slow injection) over a 24-hour period. This initial dose is normally given as 150 mg over 10 minutes (15 mg per minute), followed by 360 mg over the next 6 hours (1 mg per minute), followed by 540 mg over the remaining 18 hours (0.5 mg per minute).
 
The initial Nexterone dose may vary from person to person, depending on the individual's response to the medicine. However, doses above 2100 mg increase the risk for a potentially dangerous drop in blood pressure (hypotension), and should generally be avoided.
 
After the first 24 hours, the usual maintenance dose of 720 mg is given over 24 hours (0.5 mg per minute), which will continue until a healthcare provider determines the arrhythmia is adequately controlled. This usually takes two to four days; however, some people may need to continue the medicine for one to three weeks. Additional doses of Nexterone 150 mg may be necessary to treat any breakthrough episodes of arrhythmia that occur during this time.
 
Once your arrhythmia is adequately controlled, your healthcare provider may recommend you start taking amiodarone tablets (Cordarone®, Pacerone®). Your initial recommended dosage of amiodarone tablets will depend on how long you received Nexterone. The usual recommended dosage of amiodarone tablets is as follows:
 
  • 800 mg to 1600 mg for people who received Nexterone for less than a week
  • 600 mg to 800 mg for people who received Nexterone for one to three weeks
  • 400 mg for people who received Nexterone for longer than three weeks.
